Forms

If you are an adult, please complete both the Opening Packet and the Consent and
Release Forms for Adults. 

If you are the parent of the minor child coming to therapy, please complete the forms for
the Parents of a Minor Child, including both the Opening Packet and the Consent and
Release Forms.

You may return these forms to me through my fax, the US mail, or by email if you have a
secure email method. I will need to have them in hand before our first appointment.
