I know from both personal and professional experience that choosing a therapist can be a daunting and confusing task. To gauge our fit together, I offer a free 15 minute consultation to answer any of your questions.
As you are a new patient, please fill in the Opening Packet and the Consent and Release Forms. You may return these forms to me through my fax, the US mail, or by email if you have a secure email method. I will need to have them In hand before our first appointment.
I use a confidential voice mail and check messages frequently.
Once I receive your opening packet and consent and release forms, we can schedule your appointment. I offer afternoon and evening hours for your convenience.
I will send you instructions on how to enter the waiting room for my telehealth platform in which we will conduct your therapy. sessions. This platform is secure, private and HIPPA compliant for your safety and confidentiality.
I am in network with the following insurance companies:
* United. * Aetna. * Cigna
If you have insurance, before your session, it will be your responsibility to contact your insurance carrier to request the following information:
- Mental Health Benefits
- Co-Pay
- Deductible
- Authorization (if needed).
- Number of visits per fiscal or calendar year
I charge $125 for the initial assessment which can last up to an hour and 15 minutes. Subsequent sessions are from 45-55 minutes long for which the charge is $110 per session. I ask that you pay at the conclusion of each visit unless other arrangements have been made. If you are not covered by a health insurance plan, I will discuss the options of providing my counseling services at a mutually agreed upon fee. My services may also be covered by FSA and/or HSA. Personal check, MasterCard, Visa, American Express, and PayPal are accepted. There is a charge of $85 for missed appointments unless you have advised me 24 hours before your appointment or other extenuating circumstances arise.
